A. College Freshmen

1. High School Card or Form 138 (original and photocopy)

2. Birth Certificate (original and photocopy duly authenticated by NSO)

B. Transferees

1. Transfer Credential/Honorable Dismissal

2. Copy of Transcript of Records (for evaluation purposes)

3. Birth Certificate (original and photocopy duly authenticated by NSO)

C. Returning Students

1. Clearance from CPU Business Office as of the last term enrolled.

D. Cross-enrollees

1. Cross-enrolment permit from school currently enroled in.
